Overview
This NAIC group is GuideWell Mutual Holding Corporation, the Jacksonville, Florida, parent of Florida Blue, the Blue Cross and Blue Shield company in Florida, and, since 2022, of Triple-S, the largest health insurer in Puerto Rico. The group files three companies under its life and fraternal reporting: Triple-S Vida, Inc., Puerto Rico's leading life insurer; Florida Combined Life Insurance Company, Florida Blue's life, dental and disability affiliate; and Triple-S Blue, a Puerto Rico health company. The group ranked #88 among U.S. life insurance groups by 2024 individual life direct premiums written, according to NAIC market share data, with about $163.1 million in premiums and a 0.10% market share.
For an individual life insurance buyer, the brand that matters is Triple-S Vida. Florida Combined Life sells mainly group coverage through Florida Blue's employer business, while Triple-S Vida sells individual term, whole life, universal life, funeral and supplemental policies across Puerto Rico through its own representatives, local offices and authorized funeral directors.
GuideWell is a mutual holding company: it has no outside shareholders. Triple-S Management was a publicly traded company on the New York Stock Exchange until GuideWell bought it, and it now operates as a wholly owned subsidiary that keeps the Triple-S brand. As with UnitedHealthcare, the life companies are a small part of a much larger medical insurance business.
History
Triple-S traces its roots to 1959, when a group of Puerto Rico physicians and dentists founded Seguros de Servicios de Salud ("Triple-S") as a health plan; it became the island's Blue Cross Blue Shield licensee. In 1984 Triple-S entered life insurance by creating Seguros de Vida Triple-S. On January 31, 2006, Triple-S Management bought Great American Life Assurance Company of Puerto Rico, known as GA Life, and merged it with Seguros de Vida Triple-S. The combined company took the name Triple-S Vida.
On August 24, 2021, GuideWell agreed to buy all of Triple-S Management's shares for $36 each in cash, a deal valued at about $900 million. Shareholders approved it in December 2021, and the acquisition closed on February 1, 2022. Triple-S then served more than 1 million customers in Puerto Rico, nearly a third of the island's population.
On the Florida side, Florida Combined Life has long been the affiliate that issues Florida Blue's dental, life and disability coverage. Florida Blue says its small group life coverage is issued by Florida Combined Life, while other employer life, disability and supplemental products sold by Florida Blue representatives are issued by USAble Life, an independent company.
Life insurance products
Triple-S Vida's website describes a broad, traditional lineup.
Term life
- Level term for 1, 5, 10, 15, 20 or 30 years, with guaranteed renewal to age 95 regardless of health. It can be converted to a permanent policy without new evidence of insurability before age 75 (see converting term life to permanent: how the conversion option works).
- Decreasing term for 10, 15, 20 or 30 years, designed to follow a mortgage or loan balance.
- Optional riders on level term include waiver of premium, coverage for an additional insured and children, disability income, accidental death, accelerated benefits, first cancer and first heart attack diagnosis benefits, and a return-of-premium option.
Permanent life
- Whole Life - Heritage: lifetime coverage from $5,000, with issue ages up to 85 and premiums payable for 10 years, 20 years, to age 70 or for life. It builds loan values and offers lump-sum or income settlement options for beneficiaries.
- Universal life: flexible premiums and coverage, with loans and partial withdrawals from the accumulated value. Changes to the premium or death benefit can be requested after the first year.
- Endowment policies that pay the face amount if the insured is alive at 18, at 65 or after 20 years.
Simplified and guaranteed issue
- Simplified issue: a few health questions and no exam, with $5,000 to $100,000 for ages 0 to 65 or $5,000 to $50,000 for ages 0 to 85, with full coverage from the first day.
- Guaranteed issue: no exam and no rejection for health reasons for ages 35 to 85, with level premiums.
Funeral insurance
Triple-S Vida's funeral plans (Another Destination, Another Peace and Another Path) pay for defined funeral or cremation services delivered by authorized funeral directors in Puerto Rico, with the death benefit increasing by $50 a year for 20 years. They work much like preneed funeral insurance and funeral contracts; the burial insurance vs final expense vs preneed guide explains the difference from cash final expense coverage.
Other products
Triple-S Vida also sells cancer and dread disease, heart and stroke, accidental death and dismemberment, and disability policies; administers Puerto Rico's non-occupational disability insurance (SINOT) for employers; and offers individual retirement annuities under Puerto Rico's tax code and flexible premium deferred annuities with a 3.5% guaranteed minimum interest rate. Its group business includes group life, cancer and disability for employers. Florida Combined Life provides Blue Cross Blue Shield-branded group life, dental and disability coverage in Florida.
Buying and service
Triple-S Vida sells through its own representatives and offices in towns across Puerto Rico, and some plans can be bought online. Sales calls go to its sales line, and customer service is at 787-758-4888 (877-629-4888 from outside Puerto Rico), Monday to Friday from 8 a.m. to 7 p.m. and Saturday from 8 a.m. to 1 p.m. The Mi Triple-S Vida app and online portal show coverage, billing, payment history, policy values and loans, and premiums can also be paid through WhatsApp. The website is available in Spanish and English.
Florida residents interested in Florida Combined Life coverage generally get it through an employer that buys Florida Blue group benefits; it is not marketed as an individual life insurer.
Claims
Triple-S Vida posts claim instructions and forms by benefit type on its website, along with a claim-status tool. For death claims, beneficiaries can call customer service at 787-758-4888 or visit a local office; the life insurance claim checklist lists the documents insurers usually require, such as a certified death certificate and claim form. Under the company's claim rules, policies in force less than two years at the time of loss require additional medical records, reflecting the two-year the two-year contestability period. For funeral plans, the authorized funeral director provides the covered services directly.
Ratings and reputation
On July 30, 2026, AM Best affirmed Triple-S Vida's financial strength rating of A- (Excellent) and Florida Combined Life's rating of A (Excellent), both with stable outlooks. AM Best cited Triple-S Vida's very strong risk-adjusted capitalization, consistent premium growth and five straight years of profitability, while noting that its operating earnings have fluctuated. It described Florida Combined Life's balance sheet as very strong with a limited business profile. The same action affirmed Florida Blue at A+ (Superior), although AM Best noted the Florida Blue group's net losses in 2025.
Neither Triple-S Vida nor Florida Combined Life was among the brands ranked in the J.D. Power 2026 U.S. Individual Life Insurance Study. No S&P, Moody's or Fitch insurer ratings were found for the life companies.
Who it suits
Triple-S Vida suits Puerto Rico residents who want local service in Spanish, a nearby office and products designed for the island, from small funeral plans to term, whole life and endowment coverage. Florida Combined Life matters mainly to employees of Florida businesses that use Florida Blue for group benefits. Buyers on the mainland cannot purchase Triple-S Vida's individual policies.
- Strengths: AM Best A- (Excellent) for Triple-S Vida and A (Excellent) for Florida Combined Life, affirmed July 2026; backing of GuideWell, a large mutual holding company; a wide range of term lengths, conversion to age 75, and many riders; island-wide offices, an app and online purchase for some plans.
- Weaknesses: individual life sales are limited to Puerto Rico; Triple-S Vida's rating is one notch below Florida Combined Life's and its earnings have fluctuated; no J.D. Power or major-agency ratings beyond AM Best; Florida Combined Life does not sell individual life policies.
