Overview

UnitedHealth Group, the largest U.S. health insurer, is not a life insurance brand most people would name. But several of its insurance subsidiaries file as life insurers, and together they ranked #83 among U.S. life insurance groups by 2024 individual life direct premiums written, according to NAIC market share data, with about $194.7 million in premiums and a 0.12% market share.

The NAIC lists these companies in the UnitedHealth group: Freedom Life Insurance Company of America, National Foundation Life Insurance Company and Enterprise Life Insurance Company (the USHEALTH Group companies in Fort Worth, Texas); The Chesapeake Life Insurance Company and Mid-West National Life Insurance Company of Tennessee (from the former HealthMarkets); and UnitedHealthcare Insurance Company, Unimerica Insurance Company, Unimerica Life Insurance Company of New York and Optum Insurance of Ohio. Most of their business is health, supplemental health, dental and group coverage. Life insurance is a side line: small term policies sold alongside health plans.

For a shopper, there are two places where a UnitedHealth company sells individual life insurance today: USHEALTH Group, whose career agents with USHEALTH Advisors sell the LifeProtector term plan from Freedom Life, National Foundation Life and Enterprise Life, and UnitedHealthOne (uhone.com), which sells UnitedHealthcare Term Life SafeGuard underwritten by Golden Rule Insurance Company, another UnitedHealth subsidiary. The parent, UnitedHealth Group Incorporated, is based in Minnetonka, Minnesota, and trades on the New York Stock Exchange as UNH.

History

UnitedHealth built this collection of life-licensed companies through acquisitions of individual health insurers rather than by entering the life business directly.

  • Golden Rule. UnitedHealth bought Golden Rule Financial, an individual health insurer known for medical savings accounts, in 2003. Golden Rule Insurance Company now underwrites many UnitedHealthOne individual products, including the Term Life SafeGuard plan.
  • HealthMarkets. HealthMarkets, a Texas individual health insurer, owned The MEGA Life and Health Insurance Company, Mid-West National Life Insurance Company of Tennessee and The Chesapeake Life Insurance Company. UnitedHealth acquired HealthMarkets in 2019, and it became part of the Golden Rule Financial Corp. family. Chesapeake Life underwrites supplemental products once sold under the SureBridge brand and now under the UnitedHealthcare name.
  • USHEALTH Group. USHEALTH Group, a Fort Worth insurance holding company whose companies have sold health, supplemental and life coverage to self-employed people and small business owners for decades, was acquired by UnitedHealth Group in 2019. It describes itself as "a UnitedHealthcare company."

UnitedHealth Group's employer business also writes group life and disability coverage through UnitedHealthcare Insurance Company and Unimerica, which appears in benefits packages rather than as individual policies (see group life vs individual life insurance).

Life insurance products

LifeProtector (USHEALTH Group). A 10-year renewable term life plan, renewable until the insured reaches age 70. Premium options run from $10 to $50 in $5 increments, which keeps the coverage small and the budget predictable. The plan has no rider for accelerating the death benefit while you are living. It is not available in every state, and benefit amounts vary by state. It is underwritten by Freedom Life, National Foundation Life or Enterprise Life depending on the state.

UnitedHealthcare Term Life SafeGuard (Golden Rule). Sold through UnitedHealthOne in 10- or 20-year terms with no medical exam; whether a policy is issued may depend on answers to medical questions (see no-medical-exam term life insurance: how it works). Coverage can be kept to age 75. An optional critical illness rider pays a one-time cash benefit for covered diagnoses such as heart attack, life-threatening cancer, stroke, major organ transplant, paralysis, coma and renal failure; whatever the rider pays is subtracted from the life benefit. The site's own example uses a $50,000 term benefit with a $25,000 critical illness benefit, which shows the scale of coverage these plans are designed for.

Neither seller offers whole life, universal life or large-face term policies. Chesapeake Life stopped accepting new term and universal life applications in 2008; older policies remain in force.

Other products

The life plans are add-ons to the main business. USHEALTH Group sells limited-benefit health plans (Premier Advantage, Premier Choice, Secure Advantage, Health Access Plus), specified disease, accident, short-term disability income, dental and vision plans. UnitedHealthOne sells short-term medical, ACA and Medicare plans, accident, critical illness, hospital indemnity, dental and vision coverage.

Buying and service

USHEALTH products are sold through USHEALTH Advisors, LLC, whose agents are independently contracted (similar to captive agents, since they sell the group's own products), and are serviced through the MyUSHG.com member portal. USHEALTH customer service runs 7:00 a.m. to 7:00 p.m. Central Monday to Friday and 8:00 a.m. to 1:00 p.m. Saturday; the company lists a separate number for each insurer (see the contact table).

UnitedHealthOne sells by phone at 800-273-8115 and through independent brokers. Existing UnitedHealthOne customers use the uhone4me.com or myuhone.com member sites, or call 800-657-8205 (8:00 a.m. to 6:00 p.m. Eastern, Monday to Friday).

Claims

Neither USHEALTH nor UnitedHealthOne publishes a separate life claims phone line or a death claim form on its public pages. Beneficiaries should call the customer service number for the company that issued the policy (shown on the policy) and ask for the death claim packet. Expect to send a certified death certificate and a claimant statement; our claims guide explains the usual process.

Ratings and reputation

In June 2025 AM Best revised its outlook to negative and affirmed the A+ (Superior) financial strength ratings of UnitedHealth's health and dental insurance subsidiaries, which it calls UnitedHealthcare, citing expected weaker 2025 results in Medicare Advantage. The life-writing affiliates named above do not publish their own ratings on their websites, so ask for the rating of the specific issuing company before you buy. No UnitedHealth company was ranked in the J.D. Power 2026 U.S. Individual Life Insurance Study.

The group's individual sales practices have drawn regulatory scrutiny. In late December 2024 a Massachusetts Superior Court judge ordered HealthMarkets, The Chesapeake Life Insurance Company and HealthMarkets Insurance Agency to pay about $165 million (roughly $115 million in civil penalties and $50 million in restitution) for deceptive sales of supplemental health policies, finding they violated the state's consumer protection law and a 2009 consent judgment. UnitedHealth said it would appeal. The case concerned supplemental health products rather than life insurance, but it involves one of the same companies and sales channels.

Who it suits

These policies suit people who are already buying limited health or supplemental coverage from a UnitedHealth company and want a small amount of term life without an exam, for example to cover final expenses or a few years of bills. Anyone who needs a large death benefit, terms beyond 20 years or permanent coverage will find broader choices and usually lower prices per $1,000 from a dedicated life insurer; see How much life insurance do I need?.

  • Strengths: Backed by the largest U.S. health insurance group; no-exam term life with an optional critical illness rider; LifeProtector is priced in simple $10 to $50 monthly steps and renews to age 70; health, supplemental and life coverage from one agent.
  • Weaknesses: Small coverage amounts and only 10- or 20-year terms; no whole or universal life; no published life claims process or ratings for the issuing life companies; a 2024 Massachusetts ruling over deceptive supplemental-health sales by Chesapeake Life and HealthMarkets.
