Overview
Physicians Mutual is an Omaha, Nebraska, insurer that sells mostly to older adults, directly by phone, mail and online. Its life insurance is issued by Physicians Life Insurance Company, a subsidiary of the parent Physicians Mutual Insurance Company, a mutual owned by its policyholders. Together they market themselves as the "Physicians Mutual family." The parent company sells dental, supplemental health and pet insurance. Physicians Life sells life insurance and Medicare Supplement policies.
The group ranked #80 among U.S. life insurance groups by 2024 individual life direct premiums written, according to NAIC market share data, with about $202.4 million in premiums and a 0.13% market share. Physicians Mutual reports about $2.90 billion in assets at the mutual company and $1.74 billion at Physicians Life, according to figures on its website in late 2025.
Despite the name, you do not have to be a doctor to buy coverage. The company first sold to medical professionals and has sold to the general public since 1962. Its best-known life product is a guaranteed acceptance whole life policy of up to $30,000 for people aged 45 to 85, aimed at funeral and final expenses. Rob Reed is president and chief executive.
History
Physicians Mutual was founded in Omaha in 1902 by Edwin E. Elliott. The company describes its start as "around a kitchen table," and it began by selling health insurance to medical professionals by mail. Key milestones:
- 1962: Policies offered to the general public.
- 1970: Formed Physicians Life Insurance Company to sell life insurance.
- 2010s: Entered funeral preplanning, offering coverage connected with funeral homes.
- 2022: Began selling pet insurance.
The company has stayed an independent mutual, with no demutualization or sale, and has kept its headquarters at 2600 Dodge Street in Omaha. In 2019 it launched a television advertising campaign featuring actor John Michael Higgins.
Life insurance products
Physicians Life sells three kinds of individual life insurance:
- Guaranteed acceptance whole life. Up to $30,000 of modified whole life for applicants aged 45 to 85. There are no health questions or medical exam, and applicants within the age range cannot be turned down. Premiums are locked in at the age you buy, coverage lasts for life as long as premiums are paid, and the policy builds cash value you can borrow against. It is meant to cover a funeral, medical bills or other final expenses. "Modified" policies of this kind usually pay a reduced or graded benefit, often a return of premiums plus interest, if the insured dies of natural causes in the first years. Check the policy's waiting period before buying; see guaranteed issue life insurance: no health questions, with limits.
- Term life. Larger amounts of coverage for a set period at lower cost than permanent insurance. Plans and rates vary by state and are quoted by phone at 800-589-0929 or online.
- Children's whole life. Permanent coverage bought for a child that stays with them as they grow.
An accidental death and dismemberment rider can add up to $40,000 of extra benefit for a covered accidental death on the whole life policy; the company says it costs less than $11 a month. New policies come with a 31-day free look period. Physicians Mutual does not sell universal life, indexed universal life or variable life.
Products and limits vary by state, and the website asks visitors to choose their state before showing plans.
Other products
The Physicians Mutual family also sells:
- Medicare Supplement insurance (Physicians Life)
- Dental insurance, plus vision and hearing benefits, sold to individuals and families, including plans for seniors
- Cancer insurance and other supplemental health coverage
- Pet insurance for dogs and cats, since 2022
- Funeral preplanning coverage
Its forms library also lists annuity, long-term care and hospitalization claim forms for existing policyholders.
Buying and service
Physicians Mutual is a direct-response insurer. You apply online, by phone or by mail, and licensed company representatives answer questions and help choose coverage. It advertises on television and by mail; see Life insurance ads on TV: what "$9.95 a month" buys. Customers can register for an online account to pay one-time or recurring premiums, view claims and receive messages. Service forms, including beneficiary changes, ownership changes and collateral assignments, can be printed or filed online. General customer service is 800-228-9100.
Claims
Beneficiaries can download a life insurance claim form from the company's forms page or call 800-228-9100 for help. The company accepts accidental death claims by phone. Expect to provide a certified death certificate with the claim form; see how to file a life insurance claim. For a guaranteed acceptance policy, a death in the early years may pay only the limited benefit described in the policy.
Ratings and reputation
- AM Best: A+ (Superior) for both Physicians Mutual Insurance Company and Physicians Life Insurance Company. On November 13, 2025, AM Best upgraded both from A (Excellent) and raised their issuer credit ratings to "aa-" from "a+", with a stable outlook, citing the strongest level of risk-adjusted capitalization, a conservative investment portfolio and strong operating performance, while noting that new Medicare supplement sales had weighed on earnings.
- Weiss Ratings: A+ (Excellent) for Physicians Mutual Insurance Company and B+ (Good) for Physicians Life Insurance Company (June 2024), according to the company.
- The company does not publish S&P, Moody's or Fitch ratings.
- J.D. Power: Physicians Mutual was not among the brands ranked in the J.D. Power 2026 U.S. Individual Life Insurance Study.
The company reports total claims paid of $561.9 million a year at Physicians Mutual Insurance Company and $158.1 million at Physicians Life. Guaranteed acceptance coverage costs more per dollar of benefit than underwritten insurance because the insurer asks no health questions. Healthy buyers can usually get more coverage for the same premium with a simplified-issue or fully underwritten policy.
Who it suits
Physicians Mutual suits older adults who want a modest, guaranteed acceptance whole life policy to cover final expenses without an agent visit or health questions, especially those who already buy Medicare Supplement or dental coverage from the company. It suits less well younger families who need large amounts of income-replacement coverage, healthy seniors who could qualify for cheaper underwritten coverage, and anyone who wants permanent coverage above $30,000 without underwriting.
- Strengths: Policyholder-owned mutual founded in 1902; guaranteed acceptance whole life with no health questions for ages 45 to 85; buy directly without an agent; A+ (Superior) AM Best rating since November 2025; dental, Medicare Supplement and pet coverage from the same company.
- Weaknesses: Guaranteed acceptance coverage is capped at $30,000 and likely pays reduced benefits for early non-accidental deaths; a narrow life lineup with no universal life; ratings from only one of the major agencies; prices and products vary by state.
